Information Regarding Account Deletion:
By submitting this form, you are requesting the permanent deletion of your MyOPIP for Clinics account and all related data. This includes, but may not be limited to:
- Your user profile information (e.g., name, email, contact details).
- App specific data including but not limited to doctor details, patient details and other data associated with your user account
- Any other data associated with your user account
Important:
- Account deletion is irreversible. Once your account is deleted, you will not be able to recover your data.
- If you have any active subscriptions through the app, please ensure they are cancelled through the respective app store (Google Play Store or Apple App Store) before requesting account deletion, as deleting your app account may not automatically cancel store subscriptions.
- We aim to process account deletion requests within [e.g., 7-14 business days] of verification.
